Most people find the honest answer is "I don't know," and that is not a discipline problem. It is that the answer lives in a piece of software nobody tells you to build.

A duplicated alert opens the position twice. A dropped one leaves it open. Both fail quietly, and neither shows up until something is slow, which is usually the first moment the market is actually moving.
